The information, advice, and services provided by UPLIFT physical therapy are intended for general educational and therapeutic purposes only. Our physical therapy services do not replace medical diagnosis, treatment, or professional medical advice from a licensed physician or healthcare provider.

By using our services, you acknowledge that:

  1. Although Sadie is a qualified Sport Rehabilitator, they are currently a student physiotherapist and do not practice under the title of a physiotherapist. All services provided are within the scope of practice as a Sport Rehabilitator.

  2. You are responsible for consulting a qualified healthcare professional regarding any medical conditions, injuries, or concerns before beginning physical therapy.

  3. Results may vary, and UPLIFT physical therapy makes no guarantees regarding treatment outcomes.

  4. Our physical therapists and practitioners practice within their training and licensure scope but do not provide medical diagnoses or prescribe medication.

  5. Any exercises, advice, or recommendations provided should be followed at your own discretion and risk.

  6. UPLIFT physical therapy and its employees are not liable for any injuries, damages, or losses arising from the use of our services or reliance on our advice.

  7. By engaging with our services, you agree to release UPLIFT physical therapy from any legal claims related to your treatment.

Terms & Conditions - the basics

Welcome to UPLIFT physical therapy. By booking or using our services, you agree to abide by the following Terms & Conditions in accordance with UK law. Please read them carefully before proceeding.

1. Services Provided

UPLIFT physical therapyoffers the following services:

  • Massage Therapy – Includes sports massage, deep tissue massage, and relaxation techniques.

  • Personal Training – Tailored fitness programs to improve strength, mobility, and overall fitness.

  • Injury Rehabilitation – Non-medical rehabilitation techniques to support recovery from musculoskeletal injuries.

  • Consultations – Professional advice and assessments to guide injury management and performance improvement.

2. Qualifications & Scope of Practice

  • Sadie Hurst is a qualified Sports Rehabilitator and provides services within this scope.

  • Sadie is a student physiotherapist and does not practice as a licensed physiotherapist.

  • Services provided do not include medical diagnosis, prescription of medication, or treatment beyond the practitioner’s qualified expertise.

3. Booking & Appointments

  • All bookings must be made in advance via [website, phone, or email].

  • A deposit or full payment may be required to secure an appointment.

  • Late arrivals may result in a shortened session without a fee adjustment.

4. Cancellations, Refunds & Consumer Rights (Consumer Rights Act 2015)

  • Clients may cancel or reschedule up to 48 hours in advance without penalty.

  • Cancellations made within 48 hours will incur a 50% charge of the service fee.

  • No-shows will be charged 100% of the service fee with no refunds.

  • If UPLIFT Physical Therapy cancels an appointment, clients will be offered a full refund or an alternative appointment.

  • Clients have the right to request a refund for services that were not provided with reasonable care and skill, following UK consumer law.

5. Client Responsibilities & Health Considerations

  • Clients must provide accurate health information before services commence.

  • Clients must inform the practitioner of any pre-existing medical conditions, injuries, or concerns that may affect treatment.

  • Clients engaging in personal training, massage, or rehabilitation do so at their own risk and should consult a medical professional if necessary.

6. Liability & Health & Safety Compliance (Health & Safety at Work Act 1974)

  • While all reasonable care is taken, UPLIFT physical therapy is not liable for injuries, adverse reactions, or complications arising from services provided.

  • Clients accept full responsibility for their participation and acknowledge the risks involved.

  • First Aid: In case of an emergency, appropriate first aid will be provided, and emergency services will be contacted if necessary.

7. Payment, Pricing & Late Fees

  • Payments are accepted via bank transfer/contactless payment and online payments.

  • All prices are subject to change, but clients will be informed before booking.

  • Late payments may incur additional fees per the Late Payment of Commercial Debts (Interest) Act 1998.

8. Confidentiality & GDPR Compliance (UK Data Protection Act 2018 & GDPR)

  • Client personal data is stored securely and used only for service-related purposes.

  • No personal data will be shared with third parties without consent, except when required by law.

  • Clients have the right to access, correct, or request deletion of their data in compliance with GDPR.

9. Complaints & Dispute Resolution (Consumer Rights Act 2015)

  • If you have concerns about our services, please contact UPLIFT physical therapy at UPLIFTlincoln2025@outlook.com

  • Complaints will be handled fairly, professionally, and in line with UK consumer protection laws.

  • If a resolution cannot be reached, clients may refer the matter to the UK Trading Standards or the Financial Ombudsman Service where applicable.
